Report Form
The original Report Form is produced in advance of the event, copied in the required number, placed on clip boards and put into the breakout rooms. They will be used by participants to record the results of their work.
The completed Report Forms together with a cover, a table of contents, Augmentation Forms, Next Step Forms and the contact list are the basic parts of the Book of Proceedings.
Next Step Form
This Form is used to record next steps agreed on by participants during the Action Planning for specific projects. An original is produced prior to the event. A copy of the form is placed right next to each proposed project together with a fineliner.
The Next Step Forms are part of the Book of Proceedings.
Augmentation Form- typically used in multi-day events
The Augmentation Form is used to record ideas, suggestions and questions augmenting the content recorded in the Report Forms. An original is produced prior to the event and copied in the number needed. One Form is placed right next to each Report Sheet on the News Wall together with a fineliner.
The Augmentation Forms are part of the Book of Proceedings.
